Steps for a Simple and Effective Referral Process:<\/h2>\n

  1. Obtain verbal or written consent from the person being referred.<\/li>\n
  2. Complete the referral form in PDF or handwritten format. Electronic signatures are accepted. If you are a healthcare professional, we strongly encourage you to provide your contact information. <\/li>\n
  3. Include your email address on the form to receive a confirmation of receipt.<\/li>\n
  4. Upload the completed form.<\/li>\n<\/ol>\n
